Director of Health and Human Services requests approval of and authorization for the Chairman to sign the renewal of the following agreements, for the term July 1, 2015 through June 30, 2016, to provide outreach for and linkage to the Medi-Cal program and services as well as facilitate the Medi-Cal application process:

  1. Agreement No. 8018 with Cope Family Center, Inc., for a maximum of $63,499;
  2. Agreement No. 8019 with On The Move, Inc., for a maximum of $18,630;
  3. Agreement No. 8020 with Spirit of Unity in Napa, Inc., dba Puertas Abiertas, for a maximum of $33,324;
  4. Agreement No. 8021 with Community Health Initiative Napa County, Inc., for a maximum of $51,000;
  5. Agreement No. 8047 with American Canyon Family Center, Inc., for a maximum of $29,291;
  6. Agreement No. 8048 with Napa Valley Child Advocacy Network, Inc., dba Parent-Child Advocacy Network, Inc., for a maximum of $19,157; and
  7. Agreement No. 8056 with Up Valley Family Centers of Napa Valley, Inc., for a maximum of $53,741.

Director of Health and Human Services requests approval of and authorization for the Chairman to accept a grant and sign a grant agreement with the California Health Facilities Financing Authority for a maximum of $1,998,183.38, for the term December 3, 2015 through June 30, 2017 for construction of a Crisis Stabilization Unit.

Director of Health and Human Services requests approval of and authorization for the Chairman to sign an agreement with the City of Napa for a maximum of $17,500 for the term March 1, 2016 through June 30, 2016 to reimburse the City of Napa for costs associated with a contract with the National Alliance to End Homelessness for an evaluation of the shelter system.

Director of Health and Human Services requests approval of and authorization for the Chairman to sign the Acceptance of an Award from the California Department of Public Health Tuberculosis Control Branch in the amount of $19,659 for the term July 1, 2015 through June 30, 2016, to fund tuberculosis prevention and control activities.


6E

Director of Public Works requests approval of and authorization for the Chairman to sign Amendment No. 6 to Agreement No. 4562 with Franks Janitorial Service, Inc. increasing the maximum contract amount by $5,000 for a new maximum of $35,000 per fiscal year with an effective date of March 1, 2016, for janitorial services provided to County leased offices located at 994 Kaiser Road and the Young Building.

Director of Public Works requests approval of and authorization for the Chairman to sign Amendment No. 1 to Lease Agreement 160001B-16 with Calistoga Rental Homes, LLC for the term March 1, 2016 through January 6, 2017 for Suite I, with an option to renew for one additional year, increasing the amount by $425 for a new monthly maximum of $850 for space located at 1705 Washington Street, Calistoga, to house Health and Human Services Agency staff.

Director of Public Works requests adoption of a resolution temporarily closing a portion of Henry Road from 7:00 A.M. to 10:00 A.M. on Saturday, March 26, 2016, for the Carneros Vineyard Run.

6H

County Counsel requests approval of and authorization for the Chairman to sign Agreements with the following firms for the term of March 1, 2016 through June 30, 2016 with an automatic renewal for a maximum amount of $120,000 for legal services concerning election issues:

  1. Nielsen Merksamer; and 
  2. Arnold & Porter.

6I

Director of Housing and Intergovernmental Affairs requests adoption of a resolution approving and authorizing the Chairman to sign a Disposition and Development Agreement and all related documents for the sale of the County-owned property located at 535 Coombsville Road, City of Napa, to Idaho Pacific West Communities, Inc. for a total of $1,051,000 to develop 20 units of affordable housing and to enter into a loan commitment in the amount of $1,051,000 in the form of seller carryback financing at close of escrow.
